Traitement • Traitement médical – régime pauvre en ac. a. soufrés ? – Vitaminothérapie : B6, ac. Folique ? Traitement préventif ? – Arrêt / Réduction activité cycliste – Orientation vers d’autres sports Physiopathologie 1 Facteurs anatomiques - art. muscle psoas (64%) - bifurcation iliaque haute (9%) - muscle psoas hypertrophié Physiopathologie 2 Facteurs mécaniques - position sur le vélo - hyperflexion de hanche - longueur iliaque excessive - plicature - mouvement de pédalage - Peach G. Traitement chirurgical • Procedure conservative (75%) – endofibrosectomie – raccourcissement artériel – angioplastie veineuse • Revascularisation iliaque (25%) greffon veineux saphène calibré Endoscopie artérielle • Diagnostic – procédure percutanée – fréquence : 34 patients • histoire clinique évocatrice • épreuve d’effort + artériographie douteuse • Aide au geste opératoire : – confirme le diagnostic – localisation Endofibrosectomie sans plicature Endofibrosectomie avec plicature Traitement chirurgical • Procedure conservative (75%) – endofibrosectomie – raccourcissement artériel – angioplastie d’élargissement • Revascularisation iliaque (25%) greffon saphène calibré Procédures non recommandées Pontage prothétique Traitement endovasculaire PTA +/- stenting Expérience lyonnaise • 574 athlètes opérés (1991-2012) – Chirurgie conservative: 430 athlètes (42F, 388H) – Pontage veineux: 141 athlètes (21F, 120H) – PTA sans stent: 3 athletes (3H) • Lésions bilatérales: 40 (7%) • Rééducation spécifique post-opératoire Résultats • Suivi = 9.7 ± 3.2 ans • Récupération sportive = 3.7 ± 1 mois • Performance sportive à long terme – amélioration: – pas de changement: – arrêt du sport: – réoperation: 500 46 28 16 (87%) (8%) (5%) (2.7%) - Vink A, et al. Endofibrosis of the iliac arteries: an underestimated problem. Acta Chir Belg. 2004. Endofibrose artérielle Une pathologie non-athéromateuse iliofémorale du sportif de haut niveau • Le sport le plus exposé = cyclisme • L’essentiel des éléments diagnostiques est apporté par l’interrogatoire • Le traitement est toujours chirurgical et proposé à un patient motivé, prévenu des risques artériels. Conclusion • Prise en charge spécifique = centre référent • Equipe multidisciplinaire CHIRURGIE • Récupération rapide et sure • Avantages à long terme: – Bonne résistance aux étirements répétés – Bonne résistance aux conditions hémodynamiques élevées – Bonne perméabilité
