Journées de DESC – Inter-région Ouest – LA BAULE – 13 mars 2010 SYNDROME POST-PNEUMONECTOMIE & SYNDROME ORTHODEOXIE-PLATYPNEE Géraldine ALLAIN CAS CLINIQUE Patient de 51 ans Adénocarcinome du poumon droit T3N0M0 pneumonectomie droite en août 2008 chimiothérapie Janvier avril 2009 : 4 épisodes infectieux respiratoires traités par antibiothérapie en ambulatoire 5ème épisode en mai : épisode plus sévère avec dyspnée motivant son hospitalisation A J1, épisode de dyspnée au lever suspicion d’embolie pulmonaire angioTDM pulmonaire : négatif Déviation médiastinale Etirement veine pulmonaire inférieure gauche Foyer de pneumopathie gauche Etirement bronche lobaire inférieure Syndrome post-pneumonectomie Quelques jours plus tard, nouvel épisode de dyspnée au lever Suspicion syndrome orthodéoxie-platypnée Echographie trans-oesophagienne : FOP avec shunt droit-gauche permanent Repositionnement médiastinal par prothèse siliconée Suites opératoires simples Echographie de contrôle : fermeture du FO … 1 MECANISME, 2 SYNDROMES Déviation et rotation du médiastin dans la cavité de pneumonectomie Obstruction de l’arbre trachéo-bronchique par compression extrinsèque et étirement = Sd post-pneumonectomie Modification des rapports entre orifices caves et SIA shunt droit-gauche (FOP) = Sd orthodéoxie-platypnée SYNDROMES RARES Surtout des cas isolés rapportés Sd post-pneumonectomie Moins de 100 cas décrits dans la littérature anglophone depuis 1979 Quelques séries : Massachussets General Hospital, BOSTON, 2008 : 18 patients (1992-2006) St Antonius Hospital, Nieuweigen & University Medical Center, Utrecht, The Netherlands, 2007 : 20 patients (1990-2005) Incidence = 0,1-2% Sd orthodéoxie-platypnée Une cinquantaine de cas Une série française multicentrique de 36 patients publiée en 2005 EPIDEMIOLOGIE Surtout après pneumonectomie DROITE Pas de sexe ratio Sd post-pneumonectomie Plus fréquemment rapporté chez l’enfant (élasticité des tissus) Sd orthodéoxie-platypnée Exceptionnellement décrits après pneumonectomie gauche Non spécifique du postpneumonectomie (post–lobectomie droite, anévrysme de l’aorte ascendante, cirrhose, scoliose …) NB : environ 20% de FOP (séries autopsiques) PRESENTATION CLINIQUE Délai de survenue variable (quelques semaines à plusieurs années …) Sd post-pneumonectomie Sd orthodéoxie-platypnée Principaux symptômes rapportés : dyspnée, stridor, infections à répétition Mais aussi : douleurs thoraciques, Dyspnée en position debout Désaturation artérielle au lever Régression des symptômes en decubitus syncopes, hémoptysies, dysphagie, RGO, mort subite … Symptômes parfois seulement à l’effort NE PAS MECONNAITRE … Fistule bronchique Embolie pulmonaire Insuffisance cardiaque HTAP BPCO évolutive Récidive néoplasique Fibrose post-radique ou post-chimiothérapie … EXAMENS COMPLEMENTAIRES Radiographie standard Sd postpneumonectomie Tomodensitométrie Disparition de la cavité de pneumonectomie et hyper-expansion du poumon restant Fibroscopie bronchique Compression bronchique extrinsèque, trachéo-malacie Explorations fonctionnelles respiratoires Syndrome obstructif EXAMENS COMPLEMENTAIRES Sd orthodéoxie-platypnée Gazométrie artérielle debout et couché Echographie cardiaque trans-thoracique voire transoesophagienne avec épreuve de contraste Cathétérisme droit exceptionnellement nécessaire PRISE EN CHARGE Sd post-pneumonectomie Repositionnement médiastinal* Sd orthodéoxie-platypnée Fermeture du shunt* Utilisation de prothèses en silicone ou de sérum physiologique Développement de la thoracoscopie Ombrelle par voie percutanée (rapports anatomiques modifiés) Alternatives : Un cas décrit de fermeture préventive Chirurgie conventionnelle Prothèses expansibles (pédiatrie) Fixation péricardique Stents endo-bronchiques (adhérences pleurales) … *Résultats fonctionnels satisfaisants à long terme avec faible morbi-mortalité REPOSITIONNEMENT MEDIASTINAL REPOSITIONNEMENT MEDIASTINAL Complications … Luxation rétro-cardiaque de la prothèse Après repositionnement STENTS ENDO-BRONCHIQUES Compression lobaire inférieure gauche avec atélectasie Levée de l’atélectasie après stenting CONCLUSION Syndromes rares … Y penser devant une symptomatologie typique Confirmation par examens complémentaires simples Solutions thérapeutiques efficaces BIBLIOGRAPHIE Thomas Ng et al – Thoracoscopic approach for the treatment of postpneumonectomy syndrome – Ann Thorac Surg 2009;88:1015-8 Christopher Soll et al – The postpneumonectomy syndrome : clinical presentation and treatment – Eur J Cardiothorac Surg 2009;35:319-24 Kausik Bhattacharya et al – Platypnea-orthodeoxia syndrome : a rare complication after right pneumonectomy – Ann Thorac Surg 2009;88:2018-9 K.
