nin. Sa présence est liée à un âge de début du trouble plus
précoce, une augmentation de la prévalence du trouble
lié aux substances psychoactives, une aggravation des
manifestations cliniques, et une altération de la réponse
thérapeutique pour les sels de lithium et les anticonvulsi-
vants.
PERSPECTIVES
Peu d’études s’intéressent à l’effet des anticonvulsi-
vants dans les troubles anxieux. Dans la littérature, on
trouve une étude non contrôlée du valproate de sodium
dans le trouble panique (1),deux études de la prégabaline
dans la phobie sociale et le trouble anxieux généralisé (25,
27), une étude de la gabapentine dans la phobie sociale
(24), deux études du topiramate dans la phobie sociale
ou l’état de stress post-traumatique (3, 36), une étude de
la lamotrigine en complément d’antidépresseur dans l’état
de stress post traumatique (14).
Concernant les antipsychotiques, retenons l’étude ris-
péridone
versus
olanzapine dans l’état de stress post-
traumatique et le trouble obsessionnel compulsif (2, 6, 21,
33).
La place des thérapies cognitivo-comportementales est
essentielle en cas de comorbidité anxieuse.
