Presentation to the Subcomittee on Neurologiccal Diseases June 15, 2010 Présentation au Souscomité des maladies neurovasculaires le 15 juin 2010 Chronic Cerebrospinal Venous Insufficiency and Multiple Sclerosis Insuffisance véneuse cérébrospinale chronique (CCSVI) et la Sclérose en Plaques Prof. Paolo Zamboni Obstructions CCSVI: DEFINITION A syndrome characterized by flow blockages of the internal jugular and/or azygous veins (IJVsAZ) with insufficient drainage from the brain. Un syndrome d’obstruction d’une ou des veines jugulaires intérieures (IJV) et/ou de la veine azygos (AZY). AAN 2010 Venous pressure in the blocked IJVs-AZ is significantly higher in CCSVI. La pression est significativement plus élevée dans la veine affectée. JNNP 2009 EXTRACRANIAL BLOCKAGES AND BLOOD FLOW DISTURBANCES OF THE CEREBRAL VEINS ARE SIGNIFICANTLY ASSOCIATED WITH MS. La Vénographie par catheter est l’optimum. Echo colour Doppler (ECD) is an ideal tool for non-invasive screening. La technique d’échographie Doppler en couleur (ECD) est idéale pour une diagnostique sans invasion aucune. DIAGNOSIS TRAINING – FORMATION EN DIAGNOSTIQUE ECD TRAINING IS MANDATORY. Only In trained operators ECD showed a satisfactory reproducibility. UNE FORMATION EDC EST ESSENTIELLE. Seuls les techniciens ayant suivi une formation ont pu offrir des résultats reproductibles. Int Angiology, 2010 CCSVI TREATMENT Percutaneous Transluminal Angioplasty (PTA) is the only published technique. L’angioplastie percutanée transluminale (PTA) est la seule technique discutée dans des revues professionnelles. PTA is demonstrated TO BE SAFE. La PTA se fait en toute SÉCURITÉ. IJV pre post IJV pre post IJV pre post AZY pre post PTA and MS OUTCOME in the JVS STUDY PTA et son effet sur la SP dans l’ Étude JVS • Significant reduction of MS relapses • Beaucoup moins de rechutes SP • Significant reduction of active lesions • Beaucoup moins de lésions actives • Improved cognitive-motor functions • Fonctions motrices et cognitives améliorées • Significant decrease of chronic fatigue • Beaucoup moins de fatigue chronique • MORE STUDY IS NEEDED BUT TREATMENT MUST NOT WAIT FOR CLINICAL TRIALS. • LES ÉTUDES DOIVENT CONTINUER MAIS ON NE PEUT PLUS RETENIR LE TRAITEMENT EN ATTENDANT LES ESSAIS CLINIQUES. EVTMS a pilot RCT with abstract submitted to ECTRIMS Meeting Une étude pilote (EVTMS) : rapport à l’Assemblée ECTRIMS PTA was safe and well tolerated. Rate of restenosis is decreased compared to the first study (0% AZY and 29% IJV). A significant decrease (p=0.0227) in T2 lesion. Le traitement PTA : sécuritaire et bien toléré. La rechute du rétrécissement est réduite (0% AZY and 29% IJV). La lésion T2 est réduite de façon significative (p=0.0227). MY CONCLUSIONS (le français suit) CCSVI exists and is a serious obstruction, whether the patient has MS or not. CCSVI is significantly related to MS. Angioplasty corrects the blood flow from the brain. That correction helps people with MS. It is irresponsible not to proceed with angioplasty treatment of CCSVI in patients with MS, under the umbrella of clinical studies supervised by ethical committees. MES CONCLUSIONS (English on previous slide) La CCSVI existe. Elle constitue une obstruction sérieuse, que le patient ait ou non la SP. La CCSVI est liée de façon significative à la SP. L’angioplastie corrige la CCSVI. Cette correction aide les gens atteints de SP. Il est irresponsable de retenir le traitement de la CCSVI aux les patients atteints de la SP, sous l’égide d’études cliniques supervisées par des comités de déontonologie.
