Description de l’accident :
o Le 08 août 2024, 2 intervenants du contractant effectuent des mesures annuelles de rejets atmosphériques
au niveau de la cheminée du four A de l’atelier Polystyrène. Cette opération s’effectue sur un échafaudage
d’environ 3m de hauteur.
o Vers 11H10, l’échantillonnage étant fini, l’un des intervenants entreprend
de redescendre seul de l’échafaudage la plaque de trappe de cheminée
ayant servi à la prise d’échantillon, dans le cadre du repli de chantier.
Son collègue est dans la camionette en train d’effectuer des mesures sur
les échantillons.
o Il tient la plaque d’un poids de 1,5 kg dans sa main gauche.
Après avoir passé le portillon de sécurité, l’intervenant utilise l’échelle
pour descendre tenant toujours la plaque de la trappe dans la main.
o Il saisit le 1er barreau supérieur de l’échelle avec sa main libre puis pose
les pieds sur l’échelle.
o Afin de continuer sa descente, l’intervenant veut saisir le barreau
inférieur avec sa main libre. Il rate sa prise et saisit la barre de la structure
de l’échafaudage au lieu du barreau de l’échelle. Ceci le déstabilise et
provoque sa chute.
o La plaque de la trappe de cheminée est retrouvée au sol au pied de la
victime.
La victime portait l’intégralité de ses EPI, son casque avec jugulaire était
attaché. Les opérateurs présents sur zone alertent immédiatement les
secours du site. Après examen à l’infirmerie du site, la victime est transférée
à l’hôpital.

Alerte HSE
Analyse :
o Selon les premières analyses, la victime n’a pas appliqué les principes de sécurité de base puisqu’elle
descendait de l’échelle en tenant une pièce dans la main.
o Aucun moyen de levage n’était prévu pour monter ou descendre le matériel car la hauteur du plancher (3 mètres)
de l’échafaudage permettait de le faire de la main à la main avec un collègue resté au sol (pratique courante).
o Lors de l’évènement, le collègue de la victime n’était pas présent au pied de l’échafaudage car faisait l’analyse
de l’échantillon dans la camionette.
o La victime intervenait sur plusieurs points d’échantillonage en même temps.
Causes immédiates :
o La victime n’avait pas les 3 points d’appui.
o La victime tenait du matériel dans sa main gauche.
Causes Fondamentales :
o Mauvaise appréciation du risque
o Mauvaise préparation de chantier
Actions correctives :
o Safety stand down (ensemble des équipes et prestataires présents sur site) avec rappel des règles et des
interdictions concernant le travail en hauteur.
o Rappel de l’obligation de réception commune avec l’ensemble des entreprises concernées y compris le
donneur d’ordre pour validation de l’adéquation des besoins.
o Rencontre avec la Direction du contractant.
o Application du guide de réaction appropriée
Actions préventives :
o Campagne de sensibilisation sur la;base du témoignage vidéo de la victime et du second intervenant.
o Passage à la Safety Street spécifique échafaudage de tous les intervenants du contractant
o Réunions spécifiques Entreprise Prestataires pour rappel des exigences de montage, de réception, et
d’utilisation des échafaudages, en présence des préventeurs, des donneurs d’ordre TTE (présence obligatoire)

Recommandations :
o Pour les travaux en hauteur, prévoir lors de l’analyse des risques et la préparation du chantier la mise
en place et le retrait du matériel.
o Rappeler les 3 points d’appui obligatoires lors des déplacements sur échelle / crinoline.
o Avoir conscience du risque lié au travail en hauteur et ré-évaluer la situation en appliquant la minute
d’arrêt si changement

Incident description:
o On August 08, 2024, 2 members of the contractor's staff carry out annual measurements of atmospheric
emissions at the stack of furnace A in the Polystyrene plant. This operation is carried out on scaffolding
approximately 3m high
o At around 11.10am, when sampling had been completed, one of the
workers set about taking the chimney trap plate used for sampling down
from the scaffolding, as part of the site retreat. His colleague is in the
van, taking measurements on the samples.
o He holds the plate weighing 1.5 kg in his left hand. After passing
through the safety gate, the operator uses the ladder to descend, still
holding the plate in his hand.
o He grabs the 1st rung of the ladder with his free hand and places his
feet on the ladder.
o In order to continue his descent, the operator tries to grasp the bottom
rung with his free hand. He misses his grip and grabs the bar of the
scaffolding structure instead of the ladder rung. This destabilizes him
and causes him to fall.
o The chimney hatch plate is found on the floor at the victim's feet.
The victim was wearing full PPE, and his helmet with chinstrap was
fastened. The operators on site immediately alerted the site emergency
services. After examination at the site infirmary, the victim was transferred
to hospital.

Analysis:
o According to initial analyses, the victim did not apply basic safety principles, as he was climbing down the
ladder holding a workpiece in his hand.
o No lifting equipment was provided to raise or lower the equipment, as the height of the scaffolding floor (3
meters) meant that it could be done hand-over-hand with a colleague on the ground (common practice).
o At the time of the event, the victim's colleague was not present at the foot of the scaffolding, as he was
analyzing a sample in the van.
o The victim was working on several sampling points at the same time.
Immediate causes:
o The victim did not have the 3 support points.
o The victim was holding material in his left hand.
Root causes:
o Poor risk assessment
o Poor site preparation
Corrective actions:
o Safety stand down (all teams and contractors present on site) with reminder of rules and prohibitions
concerning work at height.
o Reminder of the obligation to hold a joint reception with all companies concerned, including the client, to
validate the suitability of requirements.
o Meeting with contractor's management.
o Application of the appropriate reaction guide.
Preventive actions:
o Awareness campaign based on the video testimony of the victim and his colleague
o Specific scaffolding Safety Street training for all the contractor's staff.
o Specific meetings with service providers to remind them of the requirements for erecting, receiving and using
scaffolding, in the presence of prevention officers and TTE representatives (attendance compulsory).

Recommendations:
o For work at height, plan for the installation and removal of equipment when analysing the risks and
preparing the worksite.
o Remind the 3 compulsory support points when moving on a ladder / crinoline.
o Be aware of the risks associated with working at height and reassess the situation, applying the one-
minute stop rule if the situation changes.
