médicamenteux sont également longs, toute leur vie le plus
souvent.
La poursuite du traitement au-delà de 12 mois doit être
soumise à la perception d’une amélioration du contrôle de la
maladie par le patient.
L’ITA pourrait être initiée un peu avant l’âge de 5 ans,
mais les études dans cette tranche d’âge manquent encore.
L’objectif est clairement de casser l’histoire naturelle de
la maladie et parfois de prévenir l’asthme chez un enfant
rhinitique. L’allergie aux acariens se déclare le plus souvent
relativement tôt dans la vie mais des patients plus âgés tire-
ront également prot de la mise en place d’un tel traitement:
des études ont été menés chez des patients au-delà de 60ans
et démontrent encore l’intérêt d’une ITA [22].
Enn l’ITA aux aéroallergènes peut être utile comme
option thérapeutique supplémentaire dans la dermatite
atopique. Si les résultats étaient plutôt controversés dans le
passé, des récents essais tendent à souligner son intérêt. Les
tests épicutanés sont disponibles pour les acariens et permet-
tent d’identier les sous-groupes de patients qui pourraient
répondre à cette voie thérapeutique [23].
Pour l’avenir …
• Les comprimés sublinguaux n’existent pas encore pour les
acariens contrairement aux pollens de graminées, mais ils
sont en développement très avancé.
• De même, des essais pré-cliniques sont en cours sur des
dispositifs transcutanés d’ITA aux acariens.
• Enn, des vaccins novateurs sont actuellement testés sur la
souris. Cette technique de vaccination est basée sur l’ADN
d’un allergène spécique: Der f1 [24].
5. Conclusion
La prise en charge thérapeutique de la maladie allergique
aux acariens nécessite une prise en charge globale associant
mesures d’environnement, traitement médicamenteux et
immunothérapie allergénique si nécessaire. Le patient
devra, comme dans toute maladie chronique, être informé
de sa maladie, et être partie prenante de son traitement pour
espérer obtenir les meilleurs résultats.
