Résumé
Objectifs:Développeretétudierl’impactd’unsystème
informatique de tableaux de bord de Suivi (TBS)basé
sur des recommandations.
Méthodes:Ungroupe de médecinsgénéralistesa
défini lastructure etlesfonctionnalitésdu système (rap-
pel desprocéduresetdesdatesde réalisation,visualisa-
tion synthétique desinformations). L’éditeur dulogiciel
de dossiermédical éOl’aimplémenté. Plan expérimen-
tal:étude randomisée, contrôlée,en grappes, compa-
rantéO+TBS (groupe intervention) versus éO(groupe
témoin). Population :utilisateurs de éOetleurs patients
diabétiqueset/ouhypertendus ≥25ans.Donnéesextrai-
tesdesdossiers informatisés, aprèsanonymisation,en
aveugle dugroupe de randomisation. Critère de juge-
ment: caractèreàjour du suivi pour chaque procédure,
danschaquegroupe,avantetaprèsintervention. Impact
mesuré parladifférenceabsolue d’évolution entre les
groupes, ajustée sur l’âge,le sexe,l’origine géogra-
phique etla catégorie professionnelle despatients.
Résultats :Cinquante médecinsontinclus 2715 patients.
Résultats en faveur dugroupe intervention pour 14 des
16procéduresanalysées.Différenceajustée statisti-
quement significative, chezlesdiabétiques,pour
l’IMC, l’examen despieds,l’électrocardiogramme,
l’examen dufond d’œil, chezleshypertendus pour
l’IMC etlaprotéinurie àla bandelette.
Conclusions:LesTBS facilitentl’application des
recommandationsen consultation. Ils sontfacilement
transposablesàd’autreslogiciels,pour touteslespatho-
logieschroniquesetlaprévention. L’utilisation desTBS
contribueraitàstructureretcoder uniformémentles
informationsdesdossiers médicaux quel quesoitle
logiciel,etfourniraità chaque médecin desindicateurs
de laqualité de sapratique.

Summary
Aim:To develop and evaluatethe impactof an elec-
tronicFollow-upModule (FM), based on guidelines.
Methods:Agroup of GPsdefined the structureand the
functionalitiesof the FM (reminders for recommended
follow-up procedures,dysplayof syntheticdata). The
FM wasimplemented in an electronicmedicalrecord
system (EMRS). Design:cluster randomised controlled
trial,tocompare éO+FM (intervention group) versus
éO(control group). Population:GPs,users of the
EMRS, and theirpatients withtype 2diabetesand/or
hypertension, aged ≥25. Data collection:from the
medicalrecords, afteranonymisation, and without
knowing the randomisation group of the patients.We
analyzed the up-to-datestatus of follow-up foreach
procedure in eachgroup, beforeand after the inter-
vention. The impactwasmeasured by the absolute
difference of the evolution between the groups,
adjusted forage,gender,geographicorigin and socio-
professionalgroup.
Results:Fifty GPsincluded 2715 patients.Results were
in favour of the intervention group for14 over16ana-
lyzed procedures.The adjusted differencebetween the
groupswas statistically significant,in the diabeticpop-
ulation,forBMI, feetexamination,fundi examination,
and electrocardiogram, and in the hypertensive popula-
tion,forBMI and strip screening forproteinuria.
Conclusions:The FM makeseasier to put clinical
guidelinesinto practice. The FM is simple to imple-
mentin otherEMRS, and itcanbeextended to other
conditionsand to preventivecare. The use of the FM
could contributetocode in the same way the data con-
tained in differentEMRS.Itcould provide GPs with
clinical indicators.
